ABSTRACT

A utilização de plantas medicinais como alternativa terapêutica vem atingindo um público cada vez maior. Assim, os fitoterápicos podem atuar como forma opcional de terapêutica levando em consideração o menor custo, e cujos benefícios se somam aos da terapia convencional. Com isso, o objetivo deste trabalho foi enfatizar a importância do cultivo e do uso racional de medicamentos fitoterápicos e plantas medicinais. Para tanto, foram incluídos artigos em português e inglês inseridos no banco de dados: Scielo, Google acadêmico, além de revistas eletrônicas, livros com embasamento científico de referência e base de dados do Ministério da Saúde entre os anos de 2018 a 2022. As plantas medicinais e os medicamentos fitoterápicos podem provocar efeitos adversos, interferir no efeito de medicamentos utilizados concomitantemente, além da possibilidade de causar intoxicações pela presença de contaminantes em produtos de baixa qualidade. Para a produção de bioativos, em níveis quali e quantitativamente adequados, o cultivo das plantas medicinais deve ser cuidadosamente realizado, através de técnicas adequadas para preservação do solo e plantio são essenciais, como a utilização de adubos verdes e a cobertura vegetal, que além de protegerem o solo da radiação solar, prevenir a evaporação excessiva da água e melhorar as características físicas, químicas e biológicas do sol. Esses critérios influenciam a qualidade do fitoterápico, portanto a fim de garantir a eficácia terapêutica, as plantas devem ser corretamente cultivadas, coletadas, identificadas e conservadas. Sendo assim, conclui-se que a utilização de plantas medicinais para tratamentos tem sido cada vez mais indicada pelos profissionais da saúde, ressaltando a importância do uso seguro e racional, com alerta para as suas consequências.


The use of medicinal plants as a therapeutic alternative has been reaching a growing public. Thus, herbal medicines can act as an optional form of therapy taking into account the lowest cost, and whose benefits are added to those of conventional therapy. Thus, the objective of this work was to emphasize the importance of the cultivation and rational use of herbal medicines and medicinal plants. To this end, articles in Portuguese and English inserted in the database: Scielo, Google academic, in addition to electronic journals, books with scientific reference base and database of the Ministry of Health between the years 2018 to 2022 were included. herbal medicines can cause adverse effects, interfere with the effect of medicines used concomitantly, in addition to the possibility of causing poisoning due to the presence of contaminants in low-quality products. For the production of bioactives, at qualitatively and quantitatively appropriate levels, the cultivation of medicinal plants must be carefully carried out, through techniques cultivated for soil preservation and planting are essential, such as the use of green manures and vegetation cover, which in addition to protect the soil from solar radiation, prevent excessive water evaporation and improve the physical, chemical and biological characteristics of the sun. These criteria influence the quality of the herbal medicine, therefore, in order to guarantee therapeutic efficacy, as the plants must be correctly cultivated, collected, identified and conserved. Therefore, it is concluded that the use of medicinal plants for treatments has been increasingly indicated by health professionals, emphasizing the importance of safe and rational use, with an alert to its consequences.


El uso de plantas medicinales como alternativa terapéutica ha ido alcanzando un público cada vez más amplio. Así, las plantas medicinales pueden actuar como una forma opcional de terapia teniendo en cuenta el menor coste, y cuyos beneficios se suman a los de la terapia convencional. Así, el objetivo de este trabajo fue destacar la importancia del cultivo y uso racional de las hierbas medicinales y plantas medicinales. Para ello, los artículos en portugués e Inglés insertados en la base de datos: Scielo, Google académico, además de revistas electrónicas, libros con base de referencia científica y base de datos del Ministerio de Salud entre los años 2018 a 2022 fueron incluidos. medicamentos a base de hierbas pueden causar efectos adversos, interferir con el efecto de los medicamentos utilizados concomitantemente, además de la posibilidad de causar intoxicación debido a la presencia de contaminantes en productos de baja calidad. Para la producción de bioactivos, en niveles cualitativa y cuantitativamente adecuados, el cultivo de plantas medicinales debe ser realizado cuidadosamente, siendo esenciales técnicas de preservación del suelo y de plantación, como el uso de abonos verdes y cobertura vegetal, que además de proteger el suelo de la radiación solar, evitan la evaporación excesiva del agua y mejoran las características físicas, químicas y biológicas del sol. Estos criterios influyen en la calidad de la fitoterapia, por lo que, para garantizar la eficacia terapéutica, las plantas deben cultivarse, recolectarse, identificarse y conservarse correctamente. Por lo tanto, se concluye que el uso de plantas medicinales para tratamientos ha sido cada vez más indicado por los profesionales de la salud, enfatizando la importancia del uso seguro y racional, con alerta a sus consecuencias.

Introduction: Maternal and child health are essential to public health, especially during pregnancy, where urogenital infections can affect mothers and fetuses. Sexually transmitted infections (STIs) increase obstetric risks and have complex connections with the human immunodeficiency virus ­ HIV. In Brazil, pregnant women with HIV are a growing concern, requiring focus and appropriate interventions. Objective: This study aimed to examine the clinical and epidemiological characteristics of urogenital infections in pregnant women with and without HIV and to assess whether there are notable differences between these groups. Methods: A scoping review was conducted following the Preferred Reporting Items for Systematic Reviews and Meta-Analyses ­ Extension for Scoping Reviews (PRISMA-ScR) and Joanna Briggs Institute guidelines. Databases such as Medical Literature Analysis and Retrieval System Online (MEDLINE), Literatura Latino-Americana e do Caribe em Ciências da Saúde (LILACS), and Scientific Electronic Library Online (SciELO) were explored using relevant terms. Inclusion/exclusion criteria selected nine studies for analysis. A Population, Intervention, Comparison, Outcome, and Study Design (PICOS) approach directed the search. Results: Pregnant women with HIV had a high prevalence of STIs, including Chlamydia trachomatis, Neisseria gonorrhoeae, Trichomonas vaginalis, and bacterial vaginosis. HIV infection appears to influence the risk and severity of urogenital infections. Pregnancy increases the risk of STIs, regardless of HIV status. Male partners may also influence the presence of STIs in pregnant women, especially those with HIV. Conclusion: This study highlights the association between HIV status and urogenital infections in pregnant women, indicating the need for appropriate screening and care. Prevention and treatment of STIs in pregnant women are essential for maternal and child health, regardless of HIV status. An in-depth understanding of these issues can improve public policies, clinical practices, and preventive interventions that target the overall health of these vulnerable populations.Keywords: HIV. Introdução: A saúde materna e infantil é essencial na saúde pública, especialmente durante a gravidez, quando infecções urogenitais podem afetar mães e fetos. Infecções sexualmente transmissíveis (IST) aumentam riscos obstétricos e têm conexões complexas com o vírus da imunodeficiência humana (HIV). No Brasil, gestantes com HIV são uma preocupação crescente, requerendo foco e intervenções adequadas. Objetivo: Este estudo teve como objetivo examinar as características clínicas e epidemiológicas das infecções urogenitais em mulheres grávidas com e sem HIV, avaliando se há diferenças notáveis entre esses grupos. Métodos: Uma revisão de escopo foi conduzida, seguindo as diretrizes Preferred Reporting Items for Systematic Reviews and Meta-Analyses - Extension for Scoping Reviews(PRISMA-ScR) e Joanna Briggs Institute. Bases de dados como Medical Literature Analysis and Retrieval System Online (MEDLINE), Literatura Latino-Americana e do Caribe em Ciências da Saúde (LILACS), and Scientific Electronic Library Online (SciELO) foram exploradas com termos relevantes. Os critérios de inclusão/exclusão selecionaram nove estudos para análise. Uma abordagem do tipo População, Intervenção, Comparação, Desfecho e Desenho do Estudo (PICOS) direcionou a pesquisa. Resultados: Mulheres grávidas com HIV apresentaram alta prevalência de IST, incluindo Chlamydia trachomatis, Neisseria gonorrhoeae, Trichomonas vaginalis e vaginose bacteriana. A infecção por HIV parece influenciar o risco e a gravidade das infecções urogenitais. A gravidez aumentou o risco de IST, independentemente do status de HIV. Os parceiros masculinos também podem influenciar a presença de IST em mulheres grávidas, especialmente aquelas com HIV. Conclusão: A associação entre o status de HIV e as infecções urogenitais em mulheres grávidas indica a necessidade de rastreamento e cuidado adequado. A prevenção e o tratamento de IST em gestantes são essenciais para a saúde materno-infantil, independentemente do status de HIV

ABSTRACT

ABSTRACT Poliovirus infection causes paralysis in up to 1 in 200 infected persons. The use of safe and effective inactivated poliovirus vaccines and live attenuated oral poliovirus vaccines (OPVs) means that only two pockets of wild-type poliovirus type 1 remain, in Afghanistan and Pakistan. However, OPVs can revert to virulence, causing outbreaks of circulating vaccine-derived poliovirus (cVDPV). During 2020-2022, cVDPV type 2 (cVDPV2) was responsible for 97-99% of poliomyelitis cases, mainly in Africa. Between January and August 2022, cVDPV2 was detected in sewage samples in Israel, the United Kingdom and the United States of America, where a case of acute flaccid paralysis caused by cVDPV2 also occurred. The Pan American Health Organization has warned that Brazil, the Dominican Republic, Haiti and Peru are at very high risk for the reintroduction of poliovirus and an additional eight countries in Latin America are at high risk, following dropping vaccination rates (average 80% coverage in 2022). Sabin type 2 monovalent OPV has been used to control VDPV2 outbreaks, but its use could also lead to outbreaks. To address this issue, a more genetically stable, novel OPV2 (nOPV2) was developed against cVDPV2 and in 2020 was granted World Health Organization Emergency Use Listing. Rolling out a novel vaccine under the Emergency Use Listing in mass settings to contain outbreaks requires unique local regulatory and operational preparedness.


RESUMEN La infección por poliovirus ocasiona parálisis en hasta 1 de cada 200 personas infectadas. La utilización de vacunas con poliovirus inactivados y de vacunas antipoliomielíticas orales con poliovirus vivos atenuados (OPV) seguras y eficaces ha logrado que solo queden dos focos de poliovirus salvaje de tipo 1, en Afganistán y Pakistán. Sin embargo, las vacunas con OPV pueden revertir a la virulencia y producir brotes de poliovirus circulantes de origen vacunal (cVDPV). Durante el período 2020-2022, el cVDPV de tipo 2 (cVDPV2) fue la causa del 97-99% de los casos de poliomielitis, sobre todo en África. Entre enero y agosto del 2022, se encontró el cVDPV2 en muestras de aguas residuales en Estados Unidos de América, donde se produjo un caso de parálisis flácida aguda por el cVDPV2, Israel y Reino Unido y. La Organización Panamericana de la Salud ha advertido que, tras la caída de las tasas de vacunación (con una cobertura promedio del 80% en el 2022), Brasil, Haití, Perú y República Dominicana corren un riesgo muy alto de reintroducción del poliovirus, en tanto que otros ocho países de América Latina se encuentran en una situación de alto riesgo. La OPV monovalente de tipo 2 de Sabin se ha utilizado para controlar los brotes de VDPV2, pero su empleo también podría ocasionar brotes. Para hacer frente a este problema, se creó una nueva OPV2 (nOPV2) contra el cVDPV2, genéticamente más estable, que en el 2020 se incluyó en la lista de uso en emergencias de la Organización Mundial de la Salud. El despliegue a gran escala de una nueva vacuna incluida en la lista de uso en emergencias con el fin de contener los brotes exige una extraordinaria preparación regulatoria y operativa local.


RESUMO A infecção pelo poliovírus causa paralisia em 1 de cada 200 pessoas infectadas. O uso de vacinas seguras e eficazes, tanto vacinas inativadas contra o poliovírus quanto vacinas orais contendo poliovírus atenuado (VOP), significa que restam apenas dois bolsões de poliovírus selvagem tipo 1, um no Afeganistão e outro no Paquistão. No entanto, a VOP pode reverter à virulência, causando surtos de poliovírus circulante derivado de vacina (cPVDV). No período 2020-2022, o cPVDV tipo 2 (cPVDV2) foi responsável por 97% a 99% dos casos de poliomielite, principalmente na África. Entre janeiro e agosto de 2022, o cPVDV2 foi detectado em amostras de esgoto em Israel, no Reino Unido e nos Estados Unidos da América, onde também houve um caso de paralisia flácida aguda causada pelo cPVDV2. A Organização Pan-Americana da Saúde alertou que, devido à queda nas taxas de vacinação (cobertura média de 80% em 2022), o Brasil, o Haiti, o Peru e a República Dominicana correm um risco muito alto de reintrodução do poliovírus e outros oito países da América Latina correm um risco alto. A VOP monovalente Sabin tipo 2 tem sido usada para controlar surtos de PVDV2, mas seu uso também pode levar a surtos. Para resolver esse problema, foi desenvolvida uma nova VOP2 (nVOP2), mais estável geneticamente, para combater o cPVDV2. Em 2020, a nVOP2 entrou na Lista de Uso Emergencial da Organização Mundial da Saúde. A distribuição de uma nova vacina incluída na Lista de Uso Emergencial em contextos de massa para conter surtos requer medidas originais de preparação operacional e regulatória em âmbito local.

ABSTRACT Objective: To assess the evolution of COVID-19 among Brazilian pregnant women, identifying sociodemographic and clinical predictors related to admission to ICU - Intensive Care Unit and death. Method: Cross-sectional, population-based study, carried out with a secondary database, based on data from the Influenza Epidemiological Surveillance Information System. Descriptive analysis was performed, followed by multiple linear regression with Poisson response, adopting critical p < 0.05. Results: Intensive care admission rates of 28.2% and death rates of 9.5% were identified. Region of residence, gestational trimester, number of comorbidities and respiratory signs and symptoms were associated with the risk of admission to intensive care. Age over 34 years, comorbidities, oxygen saturation equal to or less than 95%, admission to intensive care and ventilatory support, invasive or not, increased the risk of death. Conclusion: Sociodemographic and clinical predictors showed an association with hospitalization in intensive care and death of pregnant women with COVID-19.


RESUMEN Objetivo: Evaluar la evolución de la COVID-19 entre gestantes brasileñas, identificando predictores sociodemográficos y clínicos relacionados con el ingreso en unidad de cuidados intensivos y la muerte. Método: Estudio transversal, de base poblacional, realizado con una base de datos secundaria, a partir de datos del Sistema de Información de Vigilancia Epidemiológica de Influenza. Se realizó un análisis descriptivo, seguido de regresión lineal múltiple con respuesta de Poisson, adoptando una p crítica <0,05. Resultados: Se identificaron tasas de ingreso a cuidados intensivos del 28,2% y tasas de mortalidad del 9,5%. La región de residencia, el trimestre gestacional, el número de comorbilidades y los signos y síntomas respiratorios se asociaron con el riesgo de ingreso a cuidados intensivos. La edad mayor de 34 años, las comorbilidades, la saturación de oxígeno igual o inferior al 95%, el ingreso a cuidados intensivos y el soporte ventilatorio, sea invasivo o no, aumentaron el riesgo de muerte. Conclusión: Los predictores sociodemográficos y clínicos mostraron asociación con la hospitalización en cuidados intensivos y la muerte en gestantes con COVID-19.


RESUMO Objetivo: Avaliar a evolução da COVID-19 entre gestantes brasileiras, identificando-se os preditores sociodemográficos e clínicos relacionados à internação em unidade de terapia intensiva e ao óbito. Método: Estudo transversal e de base populacional, realizado com banco de dados secundários, a partir de dados do Sistema de Informação da Vigilância Epidemiológica da Gripe. Realizou-se análise descritiva, seguida de regressão linear múltipla com resposta Poisson, adotando-se p crítico <0,05. Resultados: Identificaram-se taxas de internação em terapia intensiva de 28,2% e de óbito de 9,5%. Região de residência, trimestre gestacional, número de comorbidades e sinais e sintomas respiratórios associaram-se ao risco de internação em terapia intensiva. Idade superior a 34 anos, comorbidades, saturação de oxigênio igual ou inferior a 95%, internação em terapia intensiva e suporte ventilatório, invasivo ou não, aumentaram o risco de óbito. Conclusão: Preditores sociodemográficos e clínicos mostraram associação com a hospitalização em terapia intensiva e com o óbito de gestantes com COVID-19.

Abstract Objective The aim of this study was to evaluate the best timing and feasibility of intrathecal application of sodium monosialoganglioside (GM1) after spinal cord contusion in Wistar rats as an experimental model. Methods Forty Wistar rats were submitted to contusion spinal cord injury after laminectomy. The animals were randomized and divided into four groups: Group 1 - Intrathecal application of GM1 24 hours after contusion; Group 2 - Intrathecal application of GM1 48 hours after contusion; Group 3 - intrathecal application of GM1 72 hours after contusion; Group 4 - Sham, with laminectomy and intrathecal application of 0.5 mL of 0.9% saline solution, without contusion. The recovery of locomotor function was evaluated at seven different moments by the Basso, Beattie, and Bresnahan (BBB) test. They were also assessed by the horizontal ladder, with sensory-motor behavioral assessment criteria, pre-and postoperatively. Results This experimental study showed better functional scores in the group submitted to the application of GM1, with statistically significant results, showing a mean increase when evaluated on known motor tests like the horizontal ladder and BBB, at all times of evaluation (p < 0.05), especially in group 2 (48 hours after spinal cord injury). Also, fewer mistakes and slips over the horizontal ladder were observed, and many points were achieved at the BBB scale analysis. Conclusion The study demonstrated that the intrathecal application of GM1 after spinal cord contusion in Wistar rats is feasible. The application 48 hours after the injury presented the best functional results.

Abstract Objective To describe the association of maternal and neonatal characteristics with the adherence status to the in-hospital stages of the Kangaroo-Mother Care Method - KMC (full, partial, and no-adherence). Methods Retrospective cohort study including infants < 2500 g admitted to a reference maternity hospital for the KMC in Rio de Janeiro from January to December 2018. Maternal and neonatal characteristics were distributed according to the adherence status to the KMC in-hospital stages. In the first stage, KMC is performed in Neonatal Intensive Care Unit and Conventional Neonatal Intermediate Care Unit. The second stage is completed in Kangaroo Neonatal Intermediate Care Unit. Multinomial multiple regression was performed with KMC adherence as a three-category dependent variable and maternal and neonatal characteristics as independent variables. Results Of 166 dyads, 102 (61.5%) participated in two stages. Those who did not participate in any stage (n = 52; 31.3%) had a lower level of education, a higher frequency of adverse conditions, and were more often single mothers; mothers who participated only in the first stage (n = 12; 7,2%) had more premature and sick infants. Conditions associated with adherence to the two stages compared to no adherence were: high school education (OR = 2.34; 95% CI = 1.08-5.07), presence of a partner (OR = 3.82; 95% CI = 1.7-8.61), no adverse conditions (OR = 3.54; 95% CI = 1.59-7.89) and no neonatal resuscitation (OR = 2.73; 95% CI = 1.22-6.1). Conclusions The study identified maternal and neonatal conditions associated with adherence status to the KMC. The results suggest opportunities to improve adherence.

Resumo: Este estudo objetiva resgatar os registros de práticas institucionais destinadas a crianças e adolescentes internados no antigo Hospital Colônia de Neuropsiquiatria Infantil de Oliveira (HCNPO), em Minas Gerais, no período de 1931 a 1974. Para isso, foi feito um estudo de caso a partir da análise documental de fontes primárias e bibliográficas sobre a instituição e as políticas referentes ao cuidado da loucura. Constata-se que a situação de saúde dos/as internos/as era caracterizada pela negligência e a violência do modelo asilar, com condições precárias de higiene, poucas atividades terapêuticas e deficientes medidas de socialização das crianças. Conclui-se que o HCNPO falhou em cuidar, proteger e estimular o desenvolvimento desses sujeitos, alvos de políticas que justificaram a intervenção social sobre seus corpos, tanto como "menores" quanto como "loucos".


Resumen: Este estudio tiene como objetivo recuperar los registros de prácticas institucionales hacia niños y adolescentes hospitalizados en el antiguo Hospital Colonia de Neuropsiquiatria Infantil de Oliveira (HCNPO), en Minas Gerais, de 1931 a 1974. Para eso, se realizó un estudio de caso a partir del análisis documental de fuentes primarias y bibliográficas sobre la institución y políticas relacionadas con el cuidado de la locura. Resulta que la situación de salud de los/as internos/as se caracterizó por la negligencia y la violencia del modelo de asilo, con malas condiciones de higiene, pocas actividades terapéuticas y medidas de socialización insatisfactorias para los niños. En conclusión, HCNPO no cuidó, protegió y estimuló el desarrollo de estos sujetos, blanco de políticas que justificaban la intervención social en sus cuerpos, tanto como "menores" cuánto como "locos".


Abstract: This study aims to retrieve the records of institutional practices towards children and adolescents hospitalized at the former Hospital Colônia de Neuropsiquiatria Infantil de Oliveira (HCNPO), in Minas Gerais, from 1931 to 1974. For that, a case study was made from the documentary analysis of primary and bibliographic sources on the institution and policies related to the care of madness. As it turns out, the health situation of the inmates was characterized by negligence and the violence of the asylum model, with poor hygiene conditions, few therapeutic activities and unsatisfactory socialization measures for the children. In conclusion, HCNPO failed to care for, protect and stimulate the development of these subjects, targets of policies that justified social intervention on their bodies, both as "minors" and as "mad".

Abstract Background: Transplant Renal Artery Stenosis (TRAS) is a recognized vascular complication after kidney transplantation. The overall risk predictors of TRAS are poorly understood. Methods: Retrospective analysis of patients with suspected TRAS (Doppler ultrasound PSV > 200 cm/s) who underwent angiographic study in a single center between 2007 and 2014. All patients with stenosis > 50% were considered with TRAS. Stenosis restricted in the body of the artery was also analyzed in a subgroup. Results: 274 patients were submitted to a renal angiography and 166 confirmed TRAS. TRAS group featured an older population (46.3 ± 11.0 vs. 40.9 ±14.2 years; p = 0.001), more frequent hypertensive nephropathy (30.1% vs. 15.7%; p = 0.01), higher incidence of Delayed Graft Function (DGF) (52.0% vs. 25.6%; p < 0.001) and longer Cold Ischemia Time (CIT) (21.5 ± 10.6 vs. 15.7 ± 12.9h; p < 0.001). In multivariable analyses, DGF (OR = 3.31; 95% CI 1.78-6.30; p < 0.0001) was independent risk factors for TRAS. DM and CIT showed a tendency towards TRAS. The compound discriminatory capacity of the multivariable model (AUC = 0.775; 95% CI 0.718-0.831) is significantly higher than systolic blood pressure and creatinine alone (AUC = 0.62; 95% CI 0.558-0.661). In body artery stenosis subgroup, DGF (OR = 1.86; 95% CI 1.04-3.36; p = 0.03) and Diabetes Mellitus (DM) (OR = 2.44; 95% CI 1.31-4.60; p = 0.005) were independent risk factors for TRAS. Conclusion: In our transplant population, DGF increased more than 3-fold the risk of TRAS. In the subgroup analysis, both DGF and DM increases the risk of body artery stenosis. The addition of other factors to hypertension and renal dysfunction may increase diagnostic accuracy.

ABSTRACT Many experimental models exist to better understand the necrosis of the femoral head etiology, both in terms of the species variety in which necrosis is induced and in the operative techniques used for treatment. Objective: This study has two main objectives, the first is to review the literature concerning experimental models of avascular necrosis of the growing femoral head, the second, to demonstrate the experimental pig model's reproducibility using a pilot study. Methods: This was a bibliographic review to describe the attempts over time to find the best species and technique for induction that would reproduce ischemic necrosis of the growing femoral head in humans. Simultaneously, a pilot study was performed to verify the replication of induction in pigs, the species that has more similarities with the human hip. The pilot's methodological analysis consists of conventional radiology and verification of possible anatomical, pathological changes. Results: In imaging exams; lateral sub-dislocation of the femur head and triangular appearance of the head were observed, characterizing its flattening; in macroscopic examination, the femoral head flattening with femoral neck widening and shortening was identified; in histology, the proliferation of articular cartilage with the presence of vascular granulation regenerative tissue, with osteoclasts and fibrocartilaginous tissue in the metaphyseal femoral neck region was identified. Conclusion: The experimental pig model can be used as a valuable tool for the reproducibility of anatomical, pathological changes in ischemic necrosis of the growing femoral head. The model is reproducible and feasible and can be beneficial for future studies on the anatomical pathology of necrosis of the growing femoral head. Level of Evidence III, Literature Review .


RESUMO Na tentativa de compreender melhor a etiologia da necrose da cabeça femoral, existe uma diversidade de modelos experimentais tanto no que diz respeito à variedade das espécies em que é induzida a necrose quanto nas técnicas operatórias utilizadas para o tratamento. Objetivo: Este trabalho tem fundamentalmente dois objetivos: a revisão da literatura concernente aos modelos experimentais da necrose avascular da cabeça do fêmur em crescimento e demonstrar a reprodutibilidade do modelo experimental do suíno por meio de um estudo piloto. Métodos: Foi realizada uma revisão bibliográfica descrevendo as tentativas ao longo do tempo em buscar qual seria a melhor espécie e técnica para indução que reproduzisse a necrose isquêmica da cabeça do fêmur em crescimento nos humanos. Simultaneamente foi feito um estudo piloto para verificar a replicação da indução na espécie suína, o espécime cujo quadril tem mais similaridades com o humano. A análise metodológica do piloto consiste na radiologia convencional e verificação das possíveis alterações anátomo patológicas. Resultados: Nos exames por imagem, foram observadas sub-luxação lateral da cabeça do fêmur e aparência triangular da cabeça, caracterizando o achatamento da mesma; no exame macroscópico, identificamos o achatamento da cabeça femoral com alargamento e encurtamento do colo; na histologia, identificamos a proliferação da cartilagem articular com presença de tecido regenerativo vascular de granulação, com osteoclastos e tecido fibrocartilaginoso na região metafisária do colo femoral. Conclusão: Podemos inferir que o modelo experimental suíno pode servir como ferramenta valiosa para a reprodutibilidade das alterações anátomo patológicas da necrose isquêmica da cabeça femoral em crescimento. O modelo é reprodutível e factível, servindo para estudos futuros sobre a anátomo patologia da necrose da cabeça do fêmur em crescimento. Nível de Evidência III, Revisão da Literatura .

ABSTRACT Objective: To evaluate histological changes in peripheral nerves of rats after sciatic nerve neurorrhaphy, according to the time of exposure to hyperbaric oxygen chamber treatment. Methods: Twenty-five Wistar rats were divided into 5 groups according to the amount of exposure to hyperbaric oxygen chamber treatment. Group 1 was the control and there was no use of hyperbaric oxygen chamber; group 2 received one week of therapy; group 3, two weeks; group 4, three weeks; and group 5, four weeks. After the fourth postoperative week, the animals were submitted to euthanasia and a sciatic nerve sample sent for histological analysis. Axons proximal and distal to the neurorrhaphy were counted with axonal regeneration index measurement. Results: We observed that the number of axons distal to neurorrhaphy increases with the amount of hyperbaric oxygen chamber exposure, the results were more expressive from the third week of treatment. However, the statistical analysis found no significant difference between the groups. Conclusion: The descriptive analysis suggests benefit of using hyperbaric oxygen chamber directly proportional to the time of therapy. The study, however, did not present statistically relevant results, probably due to the reduced sample size. Subsequent studies with more significant sampling would be of great value. RESUMO Objetivo: Avaliar as alterações histológicas nos nervos periféricos após neurorrafia do nervo ciático de ratos, de acordo com o tempo de exposição ao tratamento com câmara hiperbárica de oxigênio. Métodos: Vinte e cinco ratos da raça Wistar foram divididos em cinco grupos conforme o tempo de exposição ao tratamento com câmara hiperbárica de oxigênio. O grupo 1 não recebeu o tratamento; o grupo 2 recebeu uma semana de terapia; o grupo 3, duas semanas; o grupo 4, três semanas; e o grupo 5, quatro semanas. Após quatro semanas de pós-operatório, os animais foram submetidos à eutanásia e uma amostra do nervo ciático foi enviada para análise histológica. Foram feitas contagens do número de axônios proximalmente e distalmente à neurorrafia, com medição do índice de regeneração axonal. Resultados: Observamos que o aumento do número de axônios distais à neurorrafia foi diretamente proporcional ao tempo de exposição à câmara hiperbárica de oxigênio, sendo mais expressivo a partir da terceira semana de tratamento. Entretanto, a análise estatística não encontrou diferença significativa entre os grupos. Conclusão: A análise descritiva sugere benefício do uso da câmara hiperbárica de oxigênio. Porém, devido à amostra reduzida, o estudo não apresentou resultados estatisticamente relevantes, sendo necessária a realização de estudos subsequentes com amostragem mais significativa. ABSTRACT Objectives: To evaluate the functional and immunohistochemical effects of ganglioside GM1 and erythropoietin following experimental spinal cord injury. Methods: Thirty-two male BALB/c mice were subjected to experimental spinal cord injury using the NYU Impactor device and were randomly divided into the following groups: GM1 group, receiving standard ganglioside GM1 (30 mg/kg); erythropoietin group, receiving erythropoietin (1000 IU/kg); combination group, receiving both drugs; and control group, receiving saline (0.9%). Animals were evaluated according to the Basso Mouse Scale (BMS) and Hindlimb Mouse Function Score (MFS). After euthanasia, the immunohistochemistry of the medullary tissue of mice was analyzed. All animals received intraperitoneal treatment. Results: The GM1 group had higher BMS and MFS scores at the end of the experiment when compared to all other groups. The combination group had higher BMS and MFS scores than the erythropoietin and control groups. The erythropoietin group had higher BMS and MFS scores than the control group. Immunohistochemical tissue analysis showed a significant difference among groups. There was a significant increase in myelinated axons and in the myelinated axon length in the erythropoietin group when compared to the other intervention groups (p < 0.01). Conclusion: Erythropoietin and GM1 have therapeutic effects on axonal regeneration in mice subjected to experimental spinal cord injury, and administration of GM1 alone had the highest scores on the BMS and MFS scales.

ABSTRACT This prospective, randomized and double-blind study aims to compare two different protocols used for bowel preparation in patients scheduled for colonoscopy. The protocols were composed by solutions of Mannitol or sodium picosulfate combined with magnesium oxide. Patients from the proctology outpatient clinic of the General Surgery Unit of the Regional Hospital of Asa Norte (HRAN) comprised the sample of this study. Both the patients and the colonoscopist had no prior knowledge of the substance used to prepare bowel, which was randomly distributed among the participants. Both protocols demonstrated good and similar results regarding the efficiency of colon preparation, although the review of literature shows a difference in favor of preparation made with Mannitol solution regarding the colon neatness during the exam. In line with the literature, patients who used Mannitol solution had more side effects, highlighting the significant difference found for vomiting and sleep impairment. The preparation with Sodium Picosulfate with Magnesium Oxide was significantly superior in relation to the ease of ingestion perceived by the patients.


RESUMO Estudo prospectivo, randomizado e duplo-cego com o objetivo de comparar dois protocolos diferentes utilizados para o preparo de cólon em pacientes que realizaram colonoscopia: solução de Manitol; e solução de Picossulfato de Sódio combinado com Óxido de Magnésio. Para avaliar qual protocolo proporciona melhores resultados, 90 pacientes do ambulatório de proctologia da Unidade de Cirurgia Geral do Hospital Regional da Asa Norte (HRAN) compuseram a amostra deste estudo. Tanto os pacientes quanto o colonoscopista não tiveram prévio conhecimento da substância utilizada para o preparo do cólon, a qual foi distribuída randomicamente entre os participantes. Os dois protocolos demonstraram bons e semelhantes resultados acerca da eficiência do preparo do cólon, embora a literatura estudada tenha demonstrado diferença significativa em favor do preparo feito com solução de Manitol em relação a limpeza do cólon durante o exame. Em consonância com a literatura, os pacientes que utilizaram solução de Manitol apresentaram mais efeitos colaterais, destacando-se a diferença significativa encontrada para vômito e distúrbios do sono. O preparo com Picossulfato de Sódio com Óxido de Magnésio foi relevantemente superior em relação à facilidade de ingestão percebida pelos pacientes.

Abstract Objectives To study the impact of the implementation of the Pediatric Surviving Sepsis Campaign protocol on early recognition of sepsis, 1 -h treatment bundle and mortality. Methods Retrospective, single-center study, before and after the implementation of the sepsis protocol. Outcomes: sepsis recognition, compliance with the 1 -h bundle (fluid resuscitation, blood culture, antibiotics), time interval to fluid resuscitation and antibiotics administration, and mortality. Patients with febrile neutropenia were excluded. The comparisons between the periods were performed using non-parametric tests and odds ratios or relative risk were calculated. Results We studied 84 patients before and 103 after the protocol implementation. There was an increase in sepsis recognition (OR 21.5 [95% CI: 10.1-45.7]), in the compliance with the 1 -h bundle as a whole (62% x 0%), and with its three components: fluid resuscitation (OR 31.1 [95% CI: 3.9−247.2]), blood culture (OR 15.9 [95% CI: 3.9−65.2]), and antibiotics (OR 35.6 [95% CI: 8.9−143.2]). Significant reduction between sepsis recognition to fluid resuscitation (152min × 12min, p < 0.001) and to antibiotics administration (137min × 30min) also occurred. The risk of death before protocol implementation was four times greater (RR 4.1 [95% CI: 1.2-14.4]), and the absolute death risk reduction was 9%. Conclusion Even if we considered the low precision of some estimates, the lower limits of the Confidence Intervals show that the implementation of the Pediatric Surviving Sepsis Campaign guidelines alongside a qualitive assurance initiative has led to improvements in sepsis recognition, compliance with the 1 -h treatment bundle, reduction in the time interval to fluid resuscitation and antibiotics, and reduction in sepsis mortality.

Abstract Objective To describe the clinical, laboratory, and radiological characteristics, as well as the outcomes of children with MIS-C. Method Multicenter, prospective cohort study, conducted in 17 pediatric intensive care units in five states in Brazil, from March to July 2020. Patients from 1 month to 19 years who met the MIS-C diagnostic criteria were included consecutively. Results Fifty-six patients were included, with the following conditions: Kawasaki-like disease (n = 26), incomplete Kawasaki disease (n = 16), acute cardiac dysfunction (n = 10), toxic shock syndrome (n = 3), and macrophage activation syndrome (n = 1). Median age was 6.2 years (IQR 2.4−10.3), 70% were boys, 59% were non-whites, 20% had comorbidities, 48% reported a contact with COVID-19 cases, and 55% had a recent SARS-CoV-2 infection confirmed by RT-PCR and/or serology. Gastrointestinal symptoms were present in 71%, shock symptoms in 59%, and severe respiratory symptoms in less than 20%. -Dimer was increased in 80% and cardiac dysfunction markers in more than 75%. Treatment included immunoglobulin (89%); corticosteroids, antibiotics, and enoxaparin in about 50%; and oseltamivir and antifungal therapy in less than 10%. Only 11% needed invasive mechanical ventilation, with a median duration of five days (IQR 5-6.5). The median length of PICU stay was six days (IQR 5-11), and one death occurred (1.8%). Conclusions Most characteristics of the present MIS-C patients were similar to that of other cohorts. The present results may contribute to a broader understanding of SARS-CoV-2 infection in children and its short-term consequences. Long-term multidisciplinary follow-up is needed, since it is not known whether these patients will have chronic cardiac impairment or other sequelae.

Este estudo avaliou a prevalência de Fear of Missing Out (FOMO) em universitários, associando sexo, estágio de conclusão de curso e tipo de mídia acessada em uma instituição privada. A condição de FOMO foi investigada por meio do instrumento Scale of Assessment of FOMO com a participação de 311 estudantes. A prevalência de alto FOMO foi de 59,2%. No modelo final, as variáveis que se mostraram associadas à FOMO foram ser do sexo feminino, estar cursando a primeira metade do curso, sensação de FOMO quando não se checa mídias sociais, uso de WhatsApp mais de 5 vezes ao dia, uso de Messenger mais de 5 vezes ao dia, FOMO durante o trabalho e FOMO quando sozinho. As sensações e sentimentos associados ao FOMO sugerem influências na tomada de decisões em rotinas comportamentais diárias na produção da individualidade, identidade, no convívio e na interação social cotidiana.

OBJECTIVES: The intrathecal route has not yet been thoroughly standardized and evaluated in an experimental model of spinal cord injury (SCI) in Wistar rats. The objective of this study was to standardize and evaluate the effect of intradural injection in this animal model. METHOD: The animals were divided into 6 groups: 1) laminectomy and intradural catheter; 2) laminectomy, intradural catheter and infusion; 3) only SCI; 4) SCI and intradural catheter; 5) SCI, intradural catheter and infusion; and 6) control (laminectomy only). Motor evaluations were performed using the Basso, Beattie and Bresnahan (BBB) scale and the horizontal ladder test; motor evoked potentials were measured for functional evaluation, and histological evaluation was performed as well. All experimental data underwent statistical analysis. RESULTS: Regarding motor evoked potentials, the groups with experimental SCI had worse results than those without, but neither dural puncture nor the injection of intrathecal solution aggravated the effects of isolated SCI. Regarding histology, adverse tissue effects were observed in animals with SCI. On average, the BBB scores had the same statistical behaviour as the horizontal ladder results, and at every evaluated timepoint, the groups without SCI presented scored significantly better than those with SCI (p<0.05). The difference in performance on motor tests between rats with and without experimental SCI persisted from the first to the last test. CONCLUSIONS: The present work standardizes the model of intradural injection in experimental SCI in rats. Intrathecal puncture and injection did not independently cause significant functional or histological changes.
